M.M. House’s Classification of Patient’s Psychology

Dr. M.M. House has classified the patient’s psychology in 4 types:
- Class 1 – Philosophical or Reasonable
- Class 2 – Exacting or Critical
- Class 3 – Hysterical or Antagonistic
- Class 4 – Indifferent or Passive
A patient’s psychology towards dental treatment is largely shaped by their past life experiences and previous encounters with dental care. This psychological background not only influences how they perceive the treatment but also affects the dentist’s ability to deliver the best possible outcome. Understanding a patient’s mindset is therefore crucial for successful treatment planning. To address this, Dr. M.M. House classified patients into four psychological types. Let’s explore each of them in detail.
Class 1 – Philosophical or Reasonable
Class 1 types includes the following:
- Patient with no experience of wearing dentures but are not anticipating any difficulty in the treatment.
- Confident on the abilities of a dentist
- Wearing Dentures with good health and satisfactory Results
- Follows the dentist’s instructions properly
These patients have excellent prognosis.
Class 2 – Exacting or Critical
Below are the Class 2 type Patients:
- Suffering from Ill health
- Very conscious about aesthetics and efficiency of the artificial dentures
- Wearing Dentures with good health but dissatisfied with the aesthetics and function
- Doubts the abilities of a dentist
- Dissatisfied from the previous treatments
These patients are critical to manage but are dentist’s greatest supporter once they are satisfied by the treatment. Prognosis is good but patient satisfaction is critical.
Class 3 – Hysterical or Antagonistic
Class 3 includes the following:
- Bad health with negligence towards pathological conditions
- Wore Dentures but failed to manage
- Don’t trust the dentist’s abilities
- Complaining Attitude with no justification
These patients have a poor prognosis, they don’t appreciate even if they get the best possible outcome. These patients are very hard to manage.
Class 4 – Indifferent or Passive
Below are the Class 4 types:
- Patients not concerned about their oral health
- Don’t feel the need of dentures or teeth
- Highly Unco-operative
- Won’t maintain denture properly
- Don’t follow dentist’s instructions
These patients have a poor prognosis, they don’t appreciate dentist’s efforts and skills. They are highly negative towards any proposed treatment.
Key Take-Away
A dentist must always assess the patient’s psychology before proposing any treatment plan, as it plays a significant role in determining the treatment outcome. It’s essential to clearly explain the treatment process and set realistic expectations beforehand to avoid misunderstandings. Patients falling under Class 3 and Class 4 psychological types require special attention and thorough pre-treatment counselling, as they can be challenging to manage and may negatively impact the overall success of the treatment.
